Make the form effective with the first wage payment. However, I still think something in HR, even if it is a piece of paper with instructions on remember to do this and this are the most important things to watch out for and the InfoPath form location is still critical initially to get people use to the form and not making helpdesk calls.
After 6 - 9 months, you can drop the information sheet from the HR package or when they request the information from the helpdesk, just send them these instructions or a URL to tell them where.
Because some people just hate protocal and they will try their darndest to not be responsible for filling out that form. We also required management signature in order to process the form, so an assistant could not fill out the form and bring it done, it would have to be the immediate supervisor's signature on the form, even for a name change!

It will depend on your individual network and how complex a new user creation account is, but I like joejarvis the best so far, very sleek and easy to fill in. Good work! I have added my own set up doc below that includes a page to give to new user and a page to give to the Manager. Also includes information needed when staff member accounts need to be terminated. It could certainly be smoother to fill out by being able to tab to next text box but with limited time to spend on these document creations it does the job.
If anyone adapts it and makes it better I would be happy to check it out. Also includes termination info when required so its easy to quickly close accounts.
That's a really clear and well laid out form Ue Work , I could really do with using one of these forms for department heads to fill in for new hires. I have updated my template to an excel spreadsheet that populates many fields that require duplicate info automatically. Best Part - The second page can be printed out and given to the new staff member.
